Omerta ~Chinmoku no Okite~ オメルタ～沈黙の掟～ is the latest Boys Love game by Karin Entertainment.

Plot: Omerta is defined as a code of silence practiced by the Mafia; a refusal to give evidence to the police about criminal activities. Set in future Japan. Film-noir theme. Modern Tokyo is now a dystopian immigrant city ruled by crime syndicates. Stirring passions between hot-blooded men entangled in hatred, love, betrayal and loyalty. They classified it under: Love and Revenge Hard BL Game (PILP)

Note: Extract the files with winrar. Mount the iso with DaemonTools and install the game.
Extract the patch (alsignup_100723b.exe) in the folder with the omerta.exe. Then double click on alsignup_100723b.exe

To install the game: (your computer should be in japanese for the game to work correctly)
I installed the game on Windows 7, I don't know if it is different for Windows XP.

Download all the files to one folder, once everything is downloaded, extract to get .iso file.
You will get something like this:

You won't have the .data and .text file, to get them, extract alsignup_100723b.exe (Application) in the same folder. You need to extract the patch in order for it to work!

Don't patch the game until you have installed the game!

Use Daemon Tools to mount the image.
Click run.
You will get something like this:

The first option is to install, click that one.

click next (N) and agree (A) as needed.

Once you get up to this step:

Choose the 2nd one, choosing the first option won't install the game (at least for me it didn't).

Finish installing, you will get a popup saying it is finished, then AFTER you extracted the patch, (you will have the .data and the .text files) run the patch.
If success, you will get something like this:
